Background:
Bristol, North Somerset and South Gloucestershire (BNSSG) Clinical Commissioning Group, who are responsible for planning health services locally, plan to bring together Adult Community Health services across Bristol, North Somerset and South Gloucestershire into a single provider of services. Community Health services for Adults include community nursing, specialist support for people with long-term conditions such as Diabetes, and services that help to keep people at home when they become unwell (rather than go into hospital unnecessarily).
As the current providers of Adults’ Community Healthcare in Bristol and North Somerset, Bristol Community Health and North Somerset Community Partnership have decided to jointly bid for the 7 to 10 year contract and we want to work with you to design the best possible services for the future.
If you have experience of adult community health services or are caring for someone else that has, or if you are interested in supporting your local community health providers, we would encourage you to get in touch and join our group.
